We (barely) got tickets about 6 weeks before our trip, and although we don't do really do art as previously discussed, we make sure to see the big historic stuff. Besides, it was in The Da Vinci Code.
The painting is on the wall of the refectory of Santa Maria delle Grazie.

You have to go through two air-locked rooms to get to the room with the painting. That room is INCREDIBLY climate controlled - this painting is old and has been in the elements. World War II was not good to it which you can read about here.
Each group can only view the painting for 15 minutes and then you walk through two more air-locked rooms to exit. You are also not allowed to take pictures, and unlike the Sistine Chapel where everyone is sneaking pictures despite the no picture rule, there are serious docents here that are not messing around about pictures. Hence, I grabbed these from the Internet.
